All the data you provide is public data and will be made available to anyone who requests it. You are not required to complete any fields on this form; however, if you do not provide your first and last name, and either your phone number or your email address, you will not be able to participate in the rewards program. Your email address may be used to inform you of Edina Liquor specials and events.

General
Edina Liquor Bottle Club Members can accumulate rewards points (“points”) by engaging in purchasing activities. Points are recorded in each rewards member’s account and are in Edina Liquor’s sole discretion redeemable for in-store credit. At any time and in Edina Liquor’s sole discretion, Edina Liquor may: (i) determine whether you are eligible to redeem earned points; (ii) determine whether you have earned points; or (iii) adjust your points total. PRIOR TO REDEMPTION, ALL POINTS SHALL REMAIN THE SOLE PROPERTY OF EDINA LIQUOR.
All interpretations of these Terms and Conditions shall be at the sole discretion of Edina Liquor.
• This program may continue until Edina Liquor decides to terminate the program, at any time with or without notice.

• Edina Liquor reserves the right to add, modify, delete or otherwise change these terms, any of the rules, procedures, conditions, rewards or points pertaining to the program, at its sole discretion, with or without notice, even though changes may affect the value or impact of points already accumulated or rules for earning or using points.

• City of Edina employees of legal age are eligible to join this program. Edina Liquor employees are not eligible to join.
• There is no cost to join the Edina Liquor Bottle Club program. Program membership entitles members to the ability to earn points which can be redeemed for rewards. If by any means Edina Liquor fails to reward necessary points to an account, the points will be added upon Edina Liquor’s knowledge of occurrence and verification of the sale, and at Edina Liquor’s sole discretion.
• Edina Liquor reserves the right to discontinue membership for anyone who appears, in the sole discretion of Edina Liquor, to:
o Be using the program in a manner inconsistent with the Terms and Conditions or intent of the program or any portion of the program.
o Act in a manner inconsistent with local or federal laws, statutes, or ordinances.
o Have committed fraud, theft, misconduct, or dishonesty in connection with the member’s account, any other member account, or any component of the program.
o Have engaged in abusive, disruptive, inappropriate, or hostile conduct toward any Edina Liquor location, employee, representative, or member; or
o Have failed to pay outstanding accounts due to Edina Liquor.
Such discontinued membership may result in the loss of all accumulated points and the cancellation of participation and any perks and/or issued coupons or certificates. In addition to discontinuance of membership, Edina Liquor shall have the right to take appropriate administrative and/or legal action, including, without limitation, criminal prosecution, as it deems necessary.
• Edina Liquor is not responsible for sending members their point balances. Edina Liquor is not responsible for incorrect or inaccurate transcription of member contact information, for problems related to any of the equipment or programming associated with or utilized by the member, for any human error, for any interruption, deletion, omission, defect, or line failure of any telephone network or electronic transmission, for problems relating to computer equipment, software, inability to access any website or online service, for any other technical or non-technical error or malfunction, for lost, late, stolen, illegible, incomplete, garbled, misdirected, mutilated or postage due mail or email for whatever reason.
• If by any means information of members’ account is breached by a third party, Edina Liquor is by no means liable for unauthorized access to that account, access to points, or redemption of points. For any unauthorized use of membership rewards, Edina Liquor is not required to reimburse points or reward to member. If at any time a member becomes aware of any fraudulent activity, the member is responsible for contacting Edina Liquor and requesting termination of the account.
• The member is responsible for contacting Edina Liquor to notify them of any changes in personal information which may be needed for their account. Accounts can be updated in-store by staff or by the members at Edinaliquor.com by accessing their account.
Enrollment
• Only legal residents of the United States, of legal age to purchase and consume alcoholic beverages in the state of Minnesota, may join the rewards program.
• Participation in the program and the benefits of the program are offered at the sole discretion of Edina Liquor. To be eligible to enroll in the program you must have provided an active email address and phone number. Your rewards account will be linked by your phone number, email address and/or First and Last name.
• Enrollment becomes effective and acceptance of these Terms and Conditions begins when an individual successfully enrolls in the program.
• In the event an ineligible individual is enrolled, said membership will terminate immediately upon Edina Liquor’s knowledge of ineligibility and no compensation, points, coupons, certificates or other benefits will be paid or due to said individual.
Accrual of Points
• Edina Liquor Bottle Club points can be earned by any eligible purchases made at any Edina Liquor store locations.
• In the event a purchase is paid for with a gift card, the amount purchased with the gift card is not eligible for points.
• Gift certificate/gift card purchases may not be purchased with points.
• All point values are rounded up to the nearest whole number. All point values are awarded based on the pre-tax value of the items purchased, including both the dollars and cents spent.
• Points are accrued at the following rate: 1 point per $1 dollar spent.
• Periodically members will receive point accelerators to earn points faster on select products during promotional periods set at the sole discretion of Edina Liquor.
• For every 150 points earned members will receive a $5 credit to be redeemed in store only.
• Incentives and credits for points earned will expire within 45 days of earning the reward.
• Certain specials with bonus points may be added at the sole discretion of Edina Liquor.
• Members will receive special pricing on select products at the sole discretion of Edina Liquor communicated via email or text messages to members that opt into marketing communications.
• Members can earn additional incentives such as access to special purchasing events, educational classes and tastings, and exclusive access to limited and hard to find products at the sole discretion of Edina Liquor.
Membership Activity Requirements
• Members of the Edina Liquor Bottle Club can close their accounts at any time for any reason by communicating your desire to leave the program to any staff member at our retail locations or by emailing edinaliquor@edinamn.gov with desire to cancel account
• Accounts with inactivity and/or no purchase or visit history within 9 months will be closed by Edina Liquor
Use of Points and Reward Conditions
• Points have no cash value. Their sole purpose is to keep track of spending to verify eligibility for rewards. The amounts and frequency of rewards are determined at the sole discretion of Edina Liquor and can be changed at any time.
• Points earned by a member may not be sold, auctioned, bartered, brokered, purchased or otherwise transferred. Any points obtained in that manner will be considered to have been fraudulently obtained and deemed void and the membership of any member attempting to affect any such transfer may be terminated.
• Offers earned:
o Cannot be redeemed for cash, prizes or credit, have no cash value, and are not exchangeable.
o Are subject to the redemption terms and procedures specified by Edina Liquor.
• In no event will Edina Liquor be liable for the acts or inactions of a third-party, regardless of whether that party provides a service or product as part of the Edina Liquor Bottle Club Program.
• You agree that Edina Liquor does not warrant or guarantee that the rewards program or any such other products and services will be uninterrupted, without omissions or error free, or those defects will be corrected, or changes implemented. Should a member be determined, within the sole discretion of Edina Liquor, to have been improperly denied a reward, liability, if any, will be limited to the equivalent value of that reward as determined solely by Edina Liquor.
• Points earned may not be redeemed the same day they are earned.
Communication and Use of Contact Information
By enrolling in the Edina Liquor Bottle Club Program, you are giving Edina Liquor permission to send you periodic communications about the program and promotional messages via the email address you provided.
• Members’ names and email addresses are submitted in store during the application process.
• Email addresses may be used to inform members of Edina Liquor specials and events.
• If at any time a member does not want to receive further emails, the member must request Edina Liquor to take their email off the mailing list.

If any provision(s) of these terms and conditions are held to be invalid or unenforceable, all remaining provisions hereof will remain in full force and effect. The failure by Edina Liquor to exercise rights hereunder upon the occurrence of any of the contingencies set forth in these terms will not constitute a waiver of such rights.

Unredeemed Points
If a member elects to close their rewards account, or their account is terminated by Edina Liquor for any reason and the member has unredeemed points, the member will forfeit any right to those points and thus any unredeemed rewards.
